Fellowship Applicants

Applicants to the Fellowship Program must be physicians who are in or have completed an oral & maxillofacial surgery US residency accredited by the Commission on Dental Accreditation or have graduated from an international oral & maxillofacial surgery residency program.

Applications are accepted beginning August 1st of each academic year. 

To apply, please submit:
· Curriculum Vitae 
· Personal Statement 
· Three Letters of Recommendation (one must be from your Residency Program Director)
· Residency Case Log to include all years of residency
