- Applicants must meet the following criteria:
- Anticipated Pharm.D. from an ACPE accredited degree program with eligibility for Tennessee licensure by July 31 of residency year; and anticipated licensure no later than August 31 of residency year

- Curriculum Vitae
- Letter of Intent
- Pharmacy school academic transcript
- Three letters of recommendation, two of which must be from rotation preceptors
- Submit all application materials through PhORCAS by January 5
