Application Documents, submitted via PhORCAS:
- Personal statement / Letter of intent
- Curriculum vitae (CV)
- Pharmacy school official academic transcript
- Three letters of recommendation, two of which must be from rotation preceptors
Applicants must meet the following criteria:
- Anticipated Pharm.D. from an ACPE accredited degree program with eligibility for Tennessee licensure by July 31 of residency year
- Anticipated licensure no later than August 31 of residency year
