- Letter of Intent
- Curriculum vitae
- An official transcript from an ACPE-Accredited Doctor of Pharmacy Program or an ACPE-Accredited Bachelor of Science (B.S.) Program
- Applicants must be a graduate or candidate for graduation of an ACPE-accredited college of pharmacy (or one in the process of pursuing accreditation) or have a Foreign Pharmacy Graduate Equivalency Committee certificate (FPGEC) from the NABP. FPGEC status is not related to citizenship or VISA sponsorship.
- Three letters of recommendation
