Philosophy of Practice

The Pharmacy Department is responsible for assuring that patients cared for at Ascension Saint Thomas Hospital Midtown receive safe, effective and fiscally responsible medication therapy. This is accomplished through:

  • Optimized formulary process
  • Quality drug preparation/distribution systems
  • Pharmacist review of all medication orders
  • Pharmacist monitoring of patient response to therapy
  • Continuous quality improvement through reporting and evaluating medication related events
  • TEAMWORK

Scope

  • Hours of Operation:  24/7/365
  • Population Served:
    • Inpatients
    • Emergency Department patients
    • Same Day Surgery patients
    • Outpatient Infusion Center
    • All age groups, including Neonates

Practice Model

While not mutually exclusive, Pharmacy Services can be divided into Central and Distribution Services and Clinical Services.  Ascension Saint Thomas Hospital Midtown Pharmacy provides these services using a centralized distribution process in conjunction with Decentralized Pharmacist practices.

Central/Distribution Services include, but are not limited to:

  • Order Verification
  • IV room oversight/product checking
  • Chemotherapy, parenteral nutrition and IV compounding
  • Prepacking, stock refill checks
  • Narcotic refill processing
  • Non-sterile compounding

Decentralized and Clinical Services include, but are not limited to:

  • Order verification
  • Consults, including pharmacokinetics, renal dosing, TPN, Tikosyn, Clozaril
  • IV to PO conversions
  • Antimicrobial stewardship
  • Patient education and medication reconciliation
  • Ascension initiative management and therapeutic interchange
  • Participation in interdisciplinary patient care
  • Drug information to health care providers
  • Discharge counseling (Heart Failure, etc.)
