Applicants must have an Associate's degree or higher, or may qualify for the FSCJ Degree Track option (please see the "FSCJ Degree Track Information" page for details), with the successful completion of the courses listed below and an overall GPA of 3.0.

• College Level Math

• College Level Anatomy & Physiology I

• College Level Anatomy & Physiology II

• College Level Medical Terminology (for healthcare professionals)

• College Level English

• College Level History Course

• College Level Humanities Course

• College Level Physics (Physics for Liberal Arts will do)

• College Level Computer/Information Technology Course

The prospective student must submit the following items no later than March 1st to be considered for the Cohort that begins in August:

• Completed Application Form

• 3 professional references using the designated form

• All college transcripts

• Resume or personal statement letter

• Non-refundable $25 application fee

• Copy of current Cardiopulmonary Resuscitation (CPR) certification in Basic Life Support (BLS) for Healthcare Providers through The American Heart Association

How To Submit Transcripts: There are three ways one could submit Official Transcripts to the school:

1. Mail official transcripts to the address listed below, any mailed transcript must be submitted in a sealed envelope, sealed by the institution. Opened envelopes will not be accepted.
2. You may Hand-deliver your transcripts to the program, following the directions below under the payment section. Hand-delivered transcripts must be in a sealed envelope, sealed by the institution. Opened envelopes will not be accepted.
3. Request digital copies of transcripts from sites such as Parchment.com in the recipient section of the digital request, please put Alexis Cran in the name section and Alexis.Cran@ascension.org

How to submit Application Fees and Acceptance Fees

• Please submit only checks or Money Orders payable to Ascension St. Vincent's and put "Radiography Program" in the memo line.

• WE DO NOT ACCEPT CASH PAYMENTS

• You may hand-deliver your payments to the Radiography Program. Please bring it to Ascension St. Vincent’s Riverside to the Department of Medical Imaging located in the Hospital. Make sure the payment is in a sealed envelope with your name, phone number, and Attn: Alexis Cran, Radiography Program on the front of the envelope. 

• You may mail your payment to the Radiography Program, it is recommended that you obtain a U.S. Mail "Return Receipt Request". You may also email the program at Alexis.Cran@ascension.org to confirm receipt of payment. Please mail checks or money orders to the address listed below
