Ascension Family Health Center – Our Continuity clinic
Ascension Family Health Center – Our Continuity clinic
The Ascension Family Health Center serves as the cornerstone of our residents’ ambulatory education, providing a rich and diverse training environment in the heart of Milwaukee. With 24 fully equipped exam rooms and 2 dedicated procedure rooms, the clinic is designed to deliver comprehensive outpatient care while offering residents the opportunity to manage patients across the full spectrum of internal medicine. Hosting nearly 9,000 patient visits annually, the center provides a high-volume yet supportive setting where residents can hone their skills in preventive health, chronic disease management, acute ambulatory problems, and longitudinal care.
Residents are paired with dedicated faculty preceptors throughout their continuity clinic experience. These faculty members serve not only as clinical supervisors but also as mentors who provide consistent feedback and foster professional growth. Together, they guide residents through the Yale Office-Based Medicine Curriculum, a structured, case-based series that emphasizes evidence-based practice, preventive health, and patient-centered communication. The integration of this curriculum into real-world patient encounters ensures that residents graduate with the tools to excel in outpatient medicine.
The clinic’s multidisciplinary team-based model mirrors modern healthcare delivery. Residents work alongside board certified internists and family medicine physicians, advanced practice providers, registered nurses, medical assistants, social workers, radiology technologists, phlebotomists, and skilled administrative staff. With an onsite lab, and X Ray- we provide comprehensive care to our patient panel. This collaborative environment emphasizes interprofessional teamwork, teaching residents to navigate complex healthcare systems while ensuring safe, high-quality, and compassionate care for patients.
Through their continuity clinic at Ascension Family Health Center, residents develop long-term therapeutic relationships with patients, balancing acute visits with chronic disease management and preventive services. This longitudinal experience ensures that our graduates are not only clinically skilled but also deeply committed to building trust, improving health outcomes, and serving their communities with excellence.
