Copy of current Cardiopulmonary Resuscitation (CPR) for Basic Life Support (BLS) for healthcare providers by the American Heart Association (AHA)

Nonrefundable check or money order application fee ($25.00) made out to Ascension St. Vincent’s

Mail official transcripts to the address below. All official transcripts that are mailed must be sealed by the institution and the seal must be intact upon submission

Hand delivered transcripts are accepted as long as the transcript is sealed by the institution and the seal is intact upon submission

Please only submit personal checks or money orders made out to Ascension St. Vincent’s. In the memo line, please include Cardiovascular Technology Program

Applications and fees may be hand delivered to the Program Director

Please bring it to Ascension St. Vincent’s Riverside Medical Imaging Department located on the first floor in the hospital. Let the associate at the front desk of the Medical Imaging Department know the reason you are here.

Please make sure that the envelope is sealed, with your name and phone number, and ATTN: Amy Higginbotham

Please ensure that all aspects of the Application are completed correctly and accurately to avoid delays in processing your application. All required documents of the Application must be completed and collected to be considered for the interview process.
Reference Form
Applicants need to have 3 professional or educational references completed on their behalf. Family and Friend references will not be accepted.
