Welcome from the director

Welcome to the Ascension Resurrection Family Medicine Residency Program. We were established in 1977 and have trained generations of outstanding family medicine physicians. Our program strives to challenge each resident, give them the opportunity to individualize their training, and instill the confidence they need to transition to independent practice. Our graduates have gone on to pursue fellowships or start careers ranging from private practice, large group practices, rural medicine, and teaching in academic medicine.

Being in one of America's greatest cities, we have the unique opportunity to treat patients of all cultural and socioeconomic backgrounds. We are a community hospital with deep roots in our neighborhood. Our residents work in collaboration with residents from our Emergency Medicine, Obstetrics & Gynecology and Transitional Year programs. Our physicians see patients in the hospital, our outpatient clinic, nursing homes, home visits and outpatient specialty clinics. This diverse training experience teaches our residents not only the skills necessary to diagnose and treat disease, but how to inspire quality of life in their patients. We value our patients and the opportunity to serve them.

Our Family Medicine Residency Program is a true family, with dedicated faculty and staff committed to teaching residents and students in an engaging and supportive environment. As the Program Director, my goal is to create a learning environment that supports academic excellence, hard work, and the growth of our physicians. For this year’s upcoming recruitment season, we will be offering both in-person and virtual interviews. We hope that our format will provide you with helpful insight into what life is like as a resident physician at Ascension Resurrection. Our goal will be to get to know you, to capture the supportive family atmosphere here at Ascension Resurrection, and to highlight the opportunities for rigorous and comprehensive training. Historically, the “feel” of your interview day was a compilation of your in-person experiences with us, and we truly hope we capture that with our virtual interview experience. Dr. Donohue is board-certified in family medicine and holds a certificate of added qualifications in primary care sports medicine. After being in private practice, Dr. Donohue returned to Ascension Resurrection in 2013 to become a member of the faculty. Dr. Donohue strives to create the best learning environment for resident physicians.

Michael Ladewski, DO, FAAFP

Dr. Ladewski is board-certified in family medicine and holds a certificate of added qualifications in primary care sports medicine. He is also a Fellow of the American Academy of Family Physicians. Dr. Ladewski is certified by the American College of Lifestyle Medicine.

Jordan O'Neill, DO

Dr. O’Neill is board-certified in family medicine and holds a certificate of added qualifications in primary care sports medicine. The most recent addition to our faculty, Dr. O’Neill was a spirited addition to didactic education throughout his training here. Along with pictures of his newborn son, Marty, Dr. O’Neill will bring his enthusiasm for Point-of-Care Ultrasound to our curriculum.

Christopher Stephan, MD

Dr. Stephan is board-certified in Family Medicine. After twelve years in practice with Ascension Illinois, including nine years as a part-time faculty member elsewhere in the system, he decided to pursue full-time academic medicine and is thrilled to be a part of the Ascension Resurrection program.

Poonam Thaker, MD

Dr. Thaker is board-certified in family medicine and has her Certificate of Added Qualification in primary care sports medicine.

Annmarie Walsh, DO

A board-certified family medicine physician, Dr. Walsh really enjoys teaching medical students and residents. Upon completion of her residency at Ascension Resurrection, Dr. Walsh joined the faculty to pursue a career in academic medicine. Dr. Walsh strives to provide full-spectrum care of her patients and their families.

Mary Talen, PhD

Faculty Psychologist
Director, Primary Care Behavioral Health and Research

Mary R. Talen, PhD, is a licensed psychologist and the Director of Primary Care Behavioral Health and Research at the Ascension Resurrection Family Medicine Residency Program.  She graduated with her masters and doctoral degree from Columbia University in psychology and a Master’s degree in clinical psychopharmacology.  She has taught family medicine residents, medical students, family nurse practitioners and doctoral clinical psychology students in behavioral health and multi-professional teamwork for 30 years.  Dr. Talen has focused her research, teaching and clinical practice in systems-based healthcare.  She has published and presented at national and international conferences in primary care psychology and collaborative behavioral health and provider wellness.   Her current research projects are in physician wellness, training in challenging conversations; complexity team-based practice, and population-based care strategies in primary care.

Mayank Shah, MD, FAAFP

Office Preceptor, Family Medicine

Dr. Shah is a Family Physician at Ascension Resurrection. He completed his medical school training at Northwestern University Medical School and his residency at Advocate Lutheran General Hospital. He has a variety of interests including outpatient office procedures, child and women's health, Geriatric care, Alternative medicine including ancient Indian Ayurvedic Medicine, and spirituality and health. Dr. Shah has a CAQ in Hospice and Palliative medicine. Dr. Shah has worked as a faculty member in residency programs for over 10 years. He continues to see patients in the Family Practice Center and precepts for the residents.

Michael Plunkett, MD

Office Preceptor, Private Practice, Internal Medicine

Dr. Plunkett earned his medical degree from University of Illinois College of Medicine and completed a residency in internal medicine at University of Illinois Hospitals. He was a Fellow of the National Center for Medical Education, serving his fellowship in general medicine at University of Chicago. He also has an MBA from Northwestern University Kellogg School of Management, Evanston, IL. Dr. Plunkett is board certified in internal medicine, and is on the medical staff of Ascension Resurrection and other community hospitals in the neighborhood.

Dr. Plunkett enjoys caring for patients of all ages but has a special affinity for elderly patients (and they seem to like him, too). He has a passion for teaching young doctors and is involved in the family practice education program at Ascension Resurrection.

Dr. Plunkett is conversant in Spanish and Italian.

Sabrina Indyk, MD

Office Preceptor, Private Practice, Family Medicine

Sabrina Indyk, MD was born in the former West Germany to Polish parents and moved to Chicago at an early age. She received her bachelor's degree in biomedical sciences from Marquette University in Wisconsin, and received her medical degree from Jagiellonian University Medical College in Poland. Dr. Indyk completed her residency in family medicine at Ascension Resurrection, where she served as Chief Resident in her final year. Her clinical interests include women's health, adolescent medicine and pediatrics.
