Applications must be submitted through the PhORCAS system by January 3, 2025.
Letter of Intent
No letter of intent is required for the PGY1 Pharmacy Residency Program.
Supplemental Questions
In lieu of a letter of intent, applicants should complete the following two questions as a supplemental application for the Ascension St. Vincent PGY1 Pharmacy Residency Program and upload it to the PhORCAS system. Responses should be types in 12 point font, on one, single-spaced, typed page.
- Describe your transition from didactic lectures to APPE rotations. What have you learned from this experience and how will that influence your transition from pharmacy student to resident?
- What are your long term career goals and how will the PGY1 residency at Ascension St. Vincent help you to achieve these goals?

PGY1 Community-based Pharmacy Residency

Please provide a letter of intent for the PGY1 Community-based Pharmacy Residency program.
Supplemental Questions
Candidates: Please also answer the following questions in no more than 250 words per question and upload into the Phorcas portal.
1. Reflecting on your APPE rotations, please provide one piece of critical feedback you have received from one of your preceptors on a direct patient care rotation. Explain how you have incorporated this feedback into your practice and/or worked to improve in this area.

Miscellaneous
Representatives from the program will be available at the ASHP Midyear Clinical Meeting to answer questions - please reach out to the RPD to arrange a date/time. Qualified candidates will be invited to participate in virtual interviews in late January/early February 2024.
